Comparing the Efficacy of Long Acting Injectable Antipsychotics in Patients With Schizophrenia

Abstract
Objectives : Schizophrenia is a chronic disease that causes serious disabilities and a great number of social burdens. Since a variety of long-acting injectable antipsychotics are available now, we have more options to improve the clinical course of schizophrenia and reduce the associated socioeconomic burden. In this study, we compared not only the clinical effects of longacting injectable antipsychotics such as aripiprazole and paliperidone but also their impact on rehospitalization rates.

Methods : This was a retrospective study using medical records in a single hospital and participants were patients with chronic schizophrenia who were prescribed the long-acting injectable form of aripiprazole or paliperidone from April 2015 to April 2021. The scores of Clinical Global Impression ? Severity (CGI-S) and Brief Psychiatric Rating Scale (BPRS) were
collected as clinical scales, and the number of hospitalizations and length of hospitalization were recorded. The MannWhitney U test and t-test were performed to compare the difference in demographic and clinical characteristics between the two groups. In addition, repeated-measures ANOVA was performed for change of clinical effects, the number of rehospitalizations, and admission periods before and after each treatment.

Results : A total of 30 patients with schizophrenia were included, with 16 patients in the aripiprazole group and 14 in the paliperidone group. The number of hospitalizations and length of hospitalization decreased significantly after the application of long-acting injectables, and clinical symptoms improved in both groups. However, no significant differences were found between the two groups.

Conclusion : This is the first study that compared the clinical efficacy of aripiprazole and paliperidone in South Korea. Both these long-acting injectables not only improved clinical symptoms but also reduced the number of hospitalizations and length of hospitalization in patients with schizophrenia. However, no significant differences were found between the groups, although some clinical characteristics such as the number of drugs combined or adjunctive differed from one another. Further investigation with more participants from more sites is warranted for this conclusion.
